Eligibility for Weight Loss Surgery
Our program follows the National Institutes of Health guidelines, using Body Mass Index (BMI) to determine a patient's eligibility for surgery. BMI measures weight in relation to height.

For those whose insurance does not cover Lap-Band surgery, we offer a competitive self-pay discounted price of $12,750* (all covered services must be performed at Community Hospital). Please call for Sleeve Gastrectomy pricing.

The self-pay Lap-Band price includes:

  • Behavioral Health testing and counseling prior to surgery
  • Nutritional counseling prior to surgery

  • Exercise physiology counseling prior to surgery

  • Routine lab work prior to surgery

  • Surgeon, anesthesia and facility fee

  • Behavioral Health counseling for 90 day period following surgery, as needed

  • Nutritional counseling for 90 day period following surgery, as needed

  • Exercise physiology counseling for 90 day period following surgery, as needed

  • Post surgical visit with surgeon

  • Free fills for 6 months following surgery

  • 90 day membership to Gold’s Gym, with two personal trainer visits
  • Unlimited participation before and after surgery in monthly support group

Not included in the self-pay discounted price:
The initial screening assessment by the surgeon which is done to make sure the patient is a good candidate for surgery. The cost of this visit is $230.

Payment of the discounted cash price of $12,750* is due in full prior to surgery.

*Please note this is an outpatient procedure price which includes only the above. If additional (non-routine) testing is required, i.e. due to a specific health condition, or there are complications requiring additional care and/or admission to the hospital there may be additional charges.

BLIS Complication Protection Program
Our program, in association with Dr. Andrew Morse, is approved to participate in the BLIS program. Our self-pay patients who purchase this protection are covered for certain complications, within certain time frames, following surgery. Patients are then not responsible for the costs associated with the care for those complications.

This includes pre-operative and post-operative surgical care, pre-operative and post-operative psychological and nutritional counseling, financial conferencing, educational seminars and unlimited participation in support group meetings for those who have had the surgery and those considering it.
