Medical, Dental and Vision coverage is provided through CNIC. The deductibles for Plan A and B are as follows for in network:

  • $1500 deductible per family member, $3000 deductible for family
  • $4000 per person annual out-of-pocket maximum, $8000 family annual out-of-pocket maximum

  • The yearly dental benefit per person is $1500 and benefit maximum
  • There is a $50 copay per person or $150 for family

  • Vision exams are covered (every 12 months) with a $10 copay
  • Frames (every 24 months) and Contacts (every 12 months) are covered up to $130 per year
  • Lenses are covered every 12 months with a $25 copay for each lens, with a benefit up to $130 per lens

Flexible Spending Accounts
  • Health Flexible Spending Account (Pre-Tax Election Maximum: $2500)
  • Dependent Care Flexible Spending Account (Pre-Tax Election Maximum: $5000)

Hospital Sponsored Life Insurance, Short Term Disability and Retirement
  • Life and AD&D Insurance is Employer Paid, 1 x annual earnings up to $125,000, $2,000 for spouse, $2000 for each child (depending on age)
  • Short Term Disability is Employer Paid, 60% of weekly earnings, up to $1500 per week, max at 13 weeks
  • 403(b) Retirement Plan - Community Hospital contributes 1% of your gross earnings after your waiting period of 90 days. Community Hospital also offers a retirement matching program)
