All full-time and part-time employee who work a minimum of 24 hours a week are eligible for benefits the 1st of the month following 90 days of employment. Vision

$25 frames/lenses co-pay and $120 allowance every 24 months
$25 contact lens co-pay and $120 allowance
Life AD&D insurance
Class I: 2 times annual salary to $500,000
Class II: 1 time annual salary to $125,000
Spouse: 10,000
Child: $5,000 (age 1 to 19 or 21 if full-time student)

Optional Voluntary Life/AD&D Insurance
Employee: $10,000 increments to the lesser of 5 times annual salary or $500,000
Spouse: $5,000 increments to $150,000 maximum
Child: $2,500 increments to $10,000 maximum (spouse and child coverage only available if employee elects coverage on self)

Long-Term Disability Insurance
60% of monthly earnings to $10,000
90-day elimination period
SSNRA benefit duration (Social Security normal retirement age)
Flexible spending account allowable pre-tax amounts(for plan year May 2006-April 2007)
Medical: up to $4,000
Childcare: up to $5,000

Payroll
Community Hospital offers bi-weekly pay periods. Pay periods are as follows:
Every two weeks, pay periods begin on Sunday and end on Saturday.

Checks and direct deposits are available the Friday following the end of the pay period.

Direct deposits may be split between financial institutions for your convenience.

Retirement Plan
Community Hospital currently has a 403(b) plan to which employee may defer pre-tax dollars and to which the hospital makes a percentage contribution on gross earnings per pay period, after you are eligible to participate.
