Community Hospital Home Health staff is on call 24 hours a day, seven days a week. Our experienced nurses, therapists and other health professionals are specially trained to give you the very best in quality home care. We offer a full range of services:
  1. Skilled nursing: Experienced nurses work with you, your family and your physicians to coordinate and ensure the highest quality of care. Our nursing staff will thoroughly evaluate your medical needs, provide medical treatments, and coordinate the activities of other healthcare professionals. An important part of the care we provide is health education and instruction to you, your family and other caregivers about illness and measures to cope.


  2. Physical, occupational and speech therapy: Registered physical and occupational therapists assist you in regaining lost mobility, motor skills and function. They provide treatments and exercises to help relieve pain, and they teach the use of adaptive equipment to improve your independent quality of life at home. Speech/language pathologists provide services to facilitate improvement with communication, swallowing and aural rehabilitation.
  1. Home health aides: Certified nurses’ aides provide services to those requiring assistance with personal care needs such as bathing, shaving or shampooing the hair. Aides provide support with activities of daily living.


  2. Medical social work: Social workers consult on financial issues, eligibility requirements for assistance and other resources to meet short- and long-term needs. They may also help explore placement alternatives and provide counseling to address issues that may interfere with you or your family’s ability to respond to medical treatment.


  3. Professional support services: There may be a variety of special medical needs such as walkers, wheelchairs, I.V. therapy or home oxygen. We maintain a network of qualified professionals to assist with appropriate medical equipment and pharmaceutical, dietary and respiratory care needs.


Home care eligibility and insurance
We see patients based on a physician’s order. The requirements for service are that the patient be:

  1. Home-bound (unable for health reasons to leave home without assistance)

  2. In need of care on an intermittent basis from one of our primary services, which include nursing, physical therapy or speech therapy

  3. Under the care of a physician who has ordered home care
