Home health aides: Certified nurses’ aides provide services to those requiring assistance with personal care needs such as bathing, shaving or shampooing the hair. Aides provide support with activities of daily living.
Medical social work: Social workers consult on financial issues, eligibility requirements for assistance and other resources to meet short- and long-term needs. They may also help explore placement alternatives and provide counseling to address issues that may interfere with you or your family’s ability to respond to medical treatment.
Professional support services: There may be a variety of special medical needs such as walkers, wheelchairs, I.V. therapy or home oxygen. We maintain a network of qualified professionals to assist with appropriate medical equipment and pharmaceutical, dietary and respiratory care needs.
Home care eligibility and insurance
We see patients based on a physician’s order. The requirements for service are that the patient be:
• Home-bound (unable for health reasons to leave home without assistance)
• In need of care on an intermittent basis from one of our primary services, which include nursing, physical therapy or speech therapy
• Under the care of a physician who has ordered home care
