- Home health aides: Certified nurses’ aides provide services to those requiring assistance with personal care needs such as bathing, shaving or shampooing the hair. Aides provide support with activities of daily living.
- Medical social work: Social workers consult on financial issues, eligibility requirements for assistance and other resources to meet short- and long-term needs. They may also help explore placement alternatives and provide counseling to address issues that may interfere with you or your family’s ability to respond to medical treatment.
- Professional support services: There may be a variety of special medical needs such as walkers, wheelchairs, I.V. therapy or home oxygen. We maintain a network of qualified professionals to assist with appropriate medical equipment and pharmaceutical, dietary and respiratory care needs.
Home care eligibility and insurance
We see patients based on a physician’s order. The requirements for service are that the patient be:
- Home-bound (unable for health reasons to leave home without assistance)
- In need of care on an intermittent basis from one of our primary services, which include nursing, physical therapy or speech therapy
- Under the care of a physician who has ordered home care
